First name(s) go in the first name box and last name goes in the second name box.
If your family already has a current Homeschool Iowa membership, click the "I am a member" option
and type your family membership number in to confirm.
If your family does not have a current Homeschool Iowa membership, you may purchase one
by clicking the radio button in front of one of the membership options.

Upload a copy of your Iowa, Stanford, California, PSAT, SAT, or ACT test results. 

Upload a written recommendation from your pastor or church elder on church letterhead. 

Upload a record of your community involvement.
This should include areas of service in and through your church (vocal/musical performances, Christian education assistance, missions trips, etc); local volunteer efforts (roadside or neighborhood clean-up, visiting or assisting the elderly in their homes or care facilities, etc.); or other visible services to better your community. Please include some specific dates and names of organizations with whom or for whom you have performed these services.
